Required Sports Physical

Sports Physical

A current Sports Physical is required for participation in middle school sports. 

Physical: Print this physical form for your family doctor.  No student may practice or compete without a current physical.  No exceptions.

Physicals last two years, and you will upload the physical form when you register online for your sport. The physical date cannot expire before the sports season is over.

For new 6th graders:  Physicals must be dated January 1, 2024 or later.
