Required Sports Physical

A current Sports Physical is required to participate in middle school sports.

Physical Instructions:

  • Print the PHYSICAL FORM for your family doctor.

  • No student may practice or compete without a current physical — no exceptions.

  • Physicals are valid for two years. You will upload the form when registering online for your sport. The physical date must not expire before the sports season ends.

For new sixth graders: Physicals must be dated on or after January 1, 2025.
