Immunization Compliance for Fall 2008

Washington State Department of Health mandates that all students receive required immunization to attend school unless a personal or medical exemption has been signed.

If a compliant/up-to-date immunization record is not received at middle school registration, your student is NOT completely registered to start middle school in the fall of 2008.   You may bring updated immunization information to your school at registration time so that your child will not be placed on the exclusion list. If you are unsure of your student's compliance, please review the Department of Health's immunization requirement form below.

You are required to provide the school with a compliant/up-to-date immunization record prior to the beginning of the 2008-2009 school year for your student to attend school.
